The following list that has remained unclaimed for a period of more than 12 months is hereby published in terms of regulation 19 of the regulations published under Government Notice No. R. 581 of 1 September 1961, as amended by Government Notice No. R. 1474 of 22 September 1967 and Government Notice No. R. 1354 of 18
July 1975 and Government Notice No. R. 2187 of 5 October 1984 and Government Notice No. R. 223 of 19 February 1988 and Government Notices Nos. R. 316 and 317 of 25 February 1994. Any person claiming payment of any of these amounts must follow the procedure outlined below:
Claims must be lodged with the Compensation Commissioner, P. 0. Box 955, Pretoria, or at the nearest office of the Department of Labour or Magistrate within one month after the date of this notice and must be in the form of an affidavit reflecting the claimant's full name, address and Identity number, the grounds on which the claim is based and full particulars of the accident in respect of which the amount was awarded.
